Application for Service

If you are moving into our service area and would like to apply for service, please complete the following form and submit. You must provide a valid phone number so that we may call and obtain your social security number and driver’s license number. We must have that information before service can be started. If you would like to call us with the information, we can be reached at 437-3387.
 
 
Requested date must be minimum 24 hours notice. Requests will not be completed on the weekends or holidays.
